- Academic Appeals Petition - for students who wish to appeal their academic suspension for reasons other than medical issues.
If your appeal is based upon physical, psychiatric, or disability/learning disability reasons, attach a letter of recommendation from the appropriate office, as noted on page one: Schiffert, Cook, or SSD.
The next academic appeals meetings are scheduled for January 10 and January 17, 2012. Those students wishing to submit an appeal must turn in materials to Dr. Holloway no later than three days prior to the schedule meeting

- Authorization to Take Courses Elsewhere (Education Abroad)
- Exam Change
- Independent Study / Undergraduate Research
- Major Change, 2nd majors, adding minor
